Last week I attended the Advances in Rhinoplasty course held May 14th-17th at the Sheraton Chicago Hotel and Towers. This is the longest-running rhinoplasty course in the world, bringing together plastic surgeons from across the globe to discuss and debate their newest and favorite techniques.
If you are anything like my wife, you might be wondering, “how can you talk about rhinoplasty for 4 whole days?” To many patients, rhinoplasty might seem like a very simple operation. Yet most plastic surgeons consider it the most difficult procedure they perform. Rhinoplasty is hard because it embodies not only a 3-dimensional sculpting problem, but also a structural engineering problem. Perhaps the most important thing we have learned over the past 25 years is that you have to make the nose stronger. After rhinoplasty, the skin “shrink wraps” back onto the underlying cartilage and bony framework. Just like if the frame of your house isn’t strong, if the framework of the nose isn’t strong, it will collapse, thereby making the nose look unnatural and often making breathing through the nose almost impossible. Dr. Tolan and I perform a fair amount of revision rhinoplasty for patients who have undergone rhinoplasty by a different surgeon and now don’t like their “pinched” nostrils and/or inability to breathe.
One goal of rhinoplasty is to create a smooth line from the eyebrows
down the sides of the nose to the nasal tip. These are called the
brow-tip aesthetic lines. Wide or deviated nasal bones, or a very
narrow middle of the nose, can create imbalance between the top,
middle and bottom of the nose. Sometimes a “wide tip” is really an
optical illusion caused by a very narrow middle of the nose.
One of the more interesting talks I heard at the conference was the concept of using small burrs to sculpt the nasal bones. This is in contrast to osteotomies – “breaking the bones” – that rhinoplasty surgeons traditionally perform to straighten a broken nose or narrow a wide nose. I think that in the right patient, sculpting the bones with a small burr may lead to more predictable results, with less pain or bruising for the patient.
Another fascinating talk discussed how social media and “selfies” are changing our perception of the nose. We spend a lot more time looking at ourselves close up on cell phone or computer screens, forcing us to focus on our nose more. But prospective rhinoplasty patients must remember that when a camera lens is close to the face, it magnifies the things that are closest, including the nose. This is called the “fish-eye” effect. So, your nose isn’t as big as it looks when you are face-timing with your mother. I promise.
Most importantly, the conference re-affirmed my dedication to mastering this challenging and wonderful operation. The nose is in the center of the face, but it should not be the focus of our attention. A beautiful nose brings balance to the face and falls into the background, allowing us to focus on the eyes.

How Lash Enhancing Products Work
Lash enhancing products use an active ingredient that will increase the length of the growth cycle, also known as the Anagen Phase of the eyelash hairs. Through the use of Latisse, this product also affects pigmentation, making the color of lashes darker and even more beautiful.
What Products Work?
At Midwest Facial Plastic Surgery & Aesthetic Skincare we offer two lash enhancing products, Latisse and Revitalash. Both products provide wonderful results, however some prefer one over the other for various reasons.
When choosing the right product for you, it’s important to understand the differences of each. Latisse lash enhancement is a product that contains bimatoprost; which requires a prescription by one of our board-certified doctors. Bimatoprost is an ingredient which stimulates growth and has the ability to darken lashes. In addition to needing a prescription, a consent form is also required to inform you of all possible side effects and to ensure you are a good candidate to use the product. Our other product available, Revitalash, is an off-the-shelf readily available product in our clinics. Although Revitalash provides many of the same benefits as Latisse, it unfortunately does not have the ability to darken the lashes. However, some have found that their body reacts more favorably towards Revitalash as it can be less irritating to the eyes. Overall, both are great products that provide optimal results for our patients. The determination between which product to use is fully based on individual preferences and sensitivity levels. It’s important to note that both product lines are similar in cost, and our licensed aestheticians and doctors are comfortable recommending either one. Often, many choose to use both products simultaneously to maximize their beauty results.
Are Lash Enhancement Products Safe to Use?
When used in the correct manner, both lash enhancing products are extremely safe to use. Some people have reported slight sensitivity of the eyelid when initially starting treatment; though have indicated this sensitivity dissipates within a short period of time. Remember that although these lash enhancing products can provide desired results, the product must be continued in order to maintain the length and darker color lash benefits.

The 1st of May is long awaited by many as it symbolizes the end of a long winter and the beginning of warm, spring weather. I enjoy the creativity and challenge that comes with performing aesthetic facial surgery, but also really enjoy using my cosmetic surgery skills to help people look their best after cancer surgery. This woman had a small cancer in her parotid gland (a salivary gland), which is right in front of the ear. The cancer was invading the salivary duct, requiring complete removal of the salivary gland by her oncologist Dr. Gendron. The salivary gland is the size of an egg, so this left a very large depression or “divot” on the right side of the face. By moving a nearby muscle to fill the divot, and performing a mini-facelift on the right side, I was able to create near perfect facial symmetry for this patient.
These pictures were taken 3 weeks after surgery. This is a good example of how the facelift incision is well-hidden and becomes invisible after just a few weeks. You can see how the mini-facelift in the left image has tightened the skin and removed fine lines in the face and jowl. A more complete facelift as would be performed for purely aesthetic regions would achieve additional improvement in the jowl, and tighten the neck skin as well.

Dr. Garrett Griffin left the chilly Twin Cities on February 27th, 2015 to attend a conference in Las Vegas on using Bellafill for acne scarring. Acne scarring affects millions of Americans, often making people self-conscious and less confident in social and work settings. Resurfacing using Pixel (Erbium:YAG) or CO2 lasers, and facial fillers like Sculptra or Juvederm are some of the interventions that are more commonly performed for acne scarring at Midwest Facial Plastic Surgery and Aesthetic Skincare.
Bellafill is the new name for the facial filler formerly known as Artefill. Bellafill just received FDA approval to be used for acne scarring. Bellafill is made of 20% bovine collagen mixed with 80% PMMA microspheres. A skin test is required before receiving Bellafill to ensure that a person is not allergic to the bovine collagen portion. Bellafill is injected using a small needle in the same manner as other facial fillers. After injection, the body replaces the bovine collagen with a person’s own collagen, which forms around the PMMA microspheres.
Bellafill has several important differences in comparison to other facial fillers. Unlike all other fillers including Juvederm, Restylane, Belotero, Perlane, Sculptra or Radiesse, Bellafill should be considered permanent. Bellafill cannot be dissolved the way that hyaluronic acid fillers like Juvederm and Restylane can be dissolved. Rarely, in less than 1% of individuals, the immune system can have an unusual reaction to Bellafill creating chronic inflammation called a granuloma, and in these rare cases, Bellafill may need to be surgically removed. The fact that Bellafill is essentially permanent makes it a good filler for scarring, which is a problem that most people would like to have improved forever.
Not all types of acne scars are good candidates for improvement for Bellafill. Acne scars are typically divided into 3 types: icepick, box car, and rolling scars. Ice pick and box car scars are deeper than they are wide, with sharp edges and these scars are not good candidates for Bellafill. However, rolling scars which are wider than they are deep, with gentle curves at the edges, are excellent candidates for Bellafill. Other types of depressed scars, not just acne scars, can often be improved with Bellafill as well. Midwest Facial Plastic Surgery is excited to now offer Bellafill as an option for patients with acne or other types of depressed scars.

We’ve all seen the numerous advertisements featuring beautiful, hairless bodies promoting a product or service that guarantees a hair free and care free lifestyle; but is that the truth?
As a licensed aesthetician with over 15 years of experience, here are real questions I receive and the stripped down facts about laser hair reduction.
Q: Can only lighter skin tones have laser hair reduction?
A: No. Many lasers, including our Lumenis Duet laser, are safe for all skin tones.
Q: Do I have to have dark hair to have laser hair reduction?
A: Although dark hair responds the best, any hair with visible pigment should have success with laser hair reduction. The only people who would not be good candidates would be those with white, gray or red hair. This is due to a lack of pigment that the laser needs to use as its target.
Q: Should I allow my hair to grow out prior to coming in for a treatment, like with waxing?
A: No. The area that is being treated should be freshly shaved the day of or day before the treatment. The reason for this is that if the hair is long it could singe which could cause a heat injury, i.e. blisters on the surface of the skin. Also, we want all of the energy from the laser traveling to the base of the follicle, under the skin - not the tip.
Q: Can I achieve permanent results in just one treatment?
A: No. In general, most areas require a minimum of 6 treatments to achieve the desired results most people are looking for. Some areas of the body may require additional treatments due to hair density or hormones stimulating growth. Each individual is unique and may respond differently.
Overall, the truth of the matter is that the results of this treatment can be an extremely convenient and liberating for many people – some even say life-changing. If you have struggled with persistent or unwanted hair growth, consider laser hair reduction. This service can leave you not only feeling hair free, though confident as you bare skin during the warm weather seasons.

I recently had a little girl and noticed that she looks like a little old lady. But why is this? The primary reason is that both very young and very old people have too much skin for the bone underneath. As children get older and become adolescents and then young adults, the facial skeleton grows more than the skin around it, such that in our mid-20’s we achieve a “sweet spot” when the facial skeleton stretches our skin nice and tight and gives our eyes, nose, chin and neck nice definition with no visible wrinkling. As we enter our 30’s and then after, we begin losing bone again and our skin gets more stretchy so it doesn’t suck down onto the bone as well as it does in our 20’s. Eventually, we end up looking like little babies again with folds around our mouth, jowls, and extra skin above our eyes.
Aesthetic procedures target this imbalance between facial skin and bone. A facelift or eyelid lift (blepharoplasty) removes the extra skin and tightens the skin that is left behind. Facial fillers like Juvederm, Sculptra or fat grafting give volume back to the face, which is like making the facial skeleton a littler larger. This creates a more youthful bone to skin ratio, and makes a person look younger. Skin treatments like peels, skin pen or photorejuvenation (IPL) are literally the icing on the cake, evening skin pigment and texture to make a person look even younger. I use the mnemonic LFS to remind me to think about Lifting (surgery), Filling, and Skin during an aesthetic consultation to make sure I achieve the very best improvement for the patient.
