The American Society for Aesthetic Plastic Surgery (ASPS) revealed recently that Americans spent over $16 billion dollars in 2016 on plastic surgery and cosmetic treatments. Another explanation for this rise in male cosmetic surgery is the economy and the state of the business world, according to Business Insider publication. Why we need them and how to choose the best one for you!
It seems as though everyone has an opinion about eye creams; some people swear they are not needed, while others claim they can’t live without it. Does this delicate skin, that seems to show our age before other areas of our face, really demand a separate moisturizer? Many skincare professionals think so and want you to know why!
“Your eye area shows aging and damage more quickly than the rest of your face,” shares Cyndi, a licensed aesthetician with FACE Cosmetic Skincare of Midwest Facial Plastic Surgery. She continues, “The sebaceous glands around our eyes stop producing oil in our late teens and early twenties, which can lead to excessive dryness in this area and premature aging.” Cyndi goes on to say, “In addition, we use our eyes to express ourselves, so there is a lot of muscular movement that can lead to fine lines and wrinkles in this area.”

Yet, they are some of the most delicate and vulnerable features of our face, where surrounding skin oftentimes shows advanced aging and damage. How does one know when a surgical versus a non-surgical option is appropriate to keep our eyes looking as youthful and vibrant as we feel?
“The eyes are of the most inviting and memorable components of our face; unfortunately, they are also one of the first parts to show the signs of aging,” laments Dr. Christopher Tolan, a board-certified ENT/Facial Plastic Surgeon with Midwest Facial Plastic Surgery. “Patients often come to me complaining of excess skin, droopy eyelids, and puffy under eyes,” shares Dr. Tolan. He continues, “In advanced cases, patients need to tilt their head back to read a book due to heavy eyelids, and female patients even say they have difficulty applying mascara!”
Loss of elasticity, sagging, drooping and puffiness are, unfortunately, all normal signs of aging in the eye area. As time passes, our collagen and elastin (the building block of our skin that keep it full, firm and tight) breaks down causing looseness and sagging. This can lead to heavy upper & lower eyes lids, and eyebrows. We can lose this volume under the eye area as well, leading to a ‘hollowed out’ and gaunt appearance. The natural fat pad under our eye can also bulge and lend itself to a puffy, fallen and ‘tired’ look. In addition, we progressively lose bone density in this area, which can lead to a flattened appearance.
“For many of my patients, the progression of aging in the eye area is genetic,” states Dr. Tolan, “if one of your parents’ eye areas has aged a certain way, yours most likely will too, “he concludes. “Fortunately, we have both non-surgical and surgical approaches to deal with these signs of aging, regardless of it they are genetic or environmental in their origin,” continues Dr. Tolan.
Enter, dermal fillers. Often thought to only be used in the lips or cheeks, dermal fillers such as Allergan’s new Volbella, are quickly becoming industry standards in enhancing the appearance of the eyes and putting off the need for surgical intervention in some cases.
“For patients concerned about the gaunt and hollowed out appearance under their eyes, I will commonly start with a dermal filler like Volbella,” says Dr. Tolan. “In cases such as these, we simply restore the lost volume and smooth the appearance of any crepey skin,” he continues, “patients love this because they leave with results that are immediate and that last for up to 6-12 months!” He goes on, “Surgery can’t improve the appearance of fine lines around the eyes, but Botox and dermal fillers can have a dramatic affect for these subtler signs of eye aging.”
When there is simply too much skin and drooping, dermal filler will do very little to improve the appearance, and surgical intervention will be most beneficial. Blepharoplasty is the name of the surgery to remove excess skin and/or bulging from the upper or lower eyelids. During this procedure, a surgeon may remove or reposition small amounts of fat, skin and muscle to give you a more refreshed and youthful appearance. Incisions are discreetly made either to the upper contour of the upper lid, or through the inside of the lower eyelid. The results are a smoother, tighter and overall more refreshed appearance.

An Area Expert is changing the lives of patients with Facial Nerve Paralysis and the education of the physicians who treat them.

Unfortunately, this is a reality for thousands of people who suffer from Facial Nerve Paralysis. Although the exact cause of this condition can vary, the resulting negative impact on a patient’s well-being is universally devastating. Dr. Garrett Griffin, a board-certified Facial Plastic Surgeon at Midwest Facial Plastic Surgery in Woodbury and Eagan, Minnesota, was recently published in a new medical textbook, Diseases and Disorders of the Orbit and Ocular Adnexa. Released last month, he is the principal author of the chapter on The Facial Nerve. This textbook is targeted towards educating ophthalmologists and primary care physicians who often encounter patients affected by Facial Nerve Paralysis.
Considered a national expert on this condition, Dr. Griffin was sought out for his expertise in diagnosing and managing facial paralysis and facial nerve injury. “I was honored to help educate my colleagues in other specialties about this important topic,” he states. “The advancements in this field are progressing rapidly and I wanted to emphasize the emotional impact this condition can have on patients, as this allows their physicians to extend more comprehensive care,” Dr. Griffin continues.
The facial nerve is responsible for controlling all of the muscles of facial expression. As a result, when this nerve is injured, patients have difficulty with common movements such as smiling, closing their eyes, and keeping food in their mouths. This can make social interaction difficult and uncomfortable for the patient. “Studies have shown that patients with facial nerve paralysis have difficulty expressing their emotions accurately. This often leads to significant isolation and depression,” shares Dr. Griffin.
Bell’s palsy is the most common cause of facial paralysis, responsible for up to 70% of cases. The cause of Bell’s palsy is not completely understood, but its origins are likely linked to a virus that infects the facial nerve. “The infection causes the nerve to swell within its bony canal at the skull base, “explains Dr. Griffin. He continues, “The bone obviously can’t stretch, so the swelling cuts off the blood supply to the nerve fibers and the nerve, in turn, cannot function properly.”
85% of people with Bell’s palsy will recover to normal facial movement within a year, but 15% of people are left with facial tightness and unintended, or uncontrolled, facial movement, called synkinesis. Fortunately, Botox and injectable fillers like Juvederm and Voluma can be used in combination to improve facial control and symmetry in patients with synkinesis.
“Patients are overjoyed to learn that there are options available to restore balance and symmetry to their face, “says Dr. Griffin excitedly. He continues, “It is so gratifying, as a physician, to be able to use my cosmetic and surgical skills in tandem to help my patients feel more confident. Some of these options were not even available 10 years ago!”
Perhaps the most important message of Dr. Griffin’s chapter in Diseases and Disorders of the Orbit and Ocular Adnexa, is that 30% of facial paralysis cases are not caused by Bell’s palsy. In these instances, it is important to look for other, possibly more serious causes, such as Lyme disease, benign tumors, or cancer. Dr. Griffin recounts that “some of the saddest cases I have seen are patients with a tumor at their skull base who were told for years that they had Bell’s palsy, and now the cancer is much more difficult or impossible to treat. I really wanted to use this publication to help prevent this from happening in the future.”
A unique aspect of Dr. Griffin’s contribution to this textbook is that he not only provides a summary of the latest surgical techniques to help restore facial symmetry and movement in patients with permanent facial paralysis, but highlights the aesthetic procedures that can enhance the lives of these patients.
